About Clark Physical Therapy

Clark Physical Therapy (part of Adel Physical Therapy, Inc.) opened for business on April 1, 2007 with Steven Clark, PT, MHS, OCS being the sole therapist. This facility is a continuation of his private practice at the Heartland Holistic Health Center location. Since opening, patient growth has facilitated the need to bring on additional staff to accommodate the need for patient care. The practice emphasis is on chronic pain management, but will also see acute patient referrals.

Hours:
Monday – Friday
7:15 to 5:15
Our first appointment is scheduled at 7:15 a.m. and the last one is scheduled at 4:30 p.m.

Insurance:
We are participating providers for most major insurance companies. You will need to contact your insurance to verify that Adel Physical Therapy, Inc doing business as Clark Physical Therapy is a participating provider for your specific plan.

Direct Access:
In the state of Iowa, you are eligible for physical therapy with or without a physician referral if your insurance allows for it. Thus, you can be seen by a physical therapist without going to your family doctor or medical specialist.
